Mercer County Health District

The Mercer County Health District is the main source for certified copies of death certificates from 1908 to the present. They can also issue birth certificates for any Ohio birth after 1908. If you need a death record for someone who died in Mercer County, this is the office to call first. Fees run $30.00 per certified copy. They take cash, checks, money orders, and credit or debit cards.

You can order records in person, by mail, or through the county's online portal at merceroh.permitium.com. Walk-in hours are Monday through Friday, 7:30 am to 4:00 pm. For mail requests, send a completed application with your payment to the health district office. Phone orders are not accepted.
