The health department issues birth and death certificates for deaths that happened in Hamilton County outside the city limits of Cincinnati. For deaths within Cincinnati proper, you need to go to the Cincinnati Health Department at 312 Market Street.

This split between city and county health departments is important to know. If the person died inside Cincinnati, contact the city office. If they died elsewhere in Hamilton County, contact the county office. Both offices issue certified copies that are valid for legal use. The fee for a certified death certificate is around $25.00. You can request records in person or by mail. Bring a valid photo ID with you for in-person requests.
